Abstract
Johanna Makgata is a 32 year old woman who was pregnant with her second child when she presented at the antenatal clinic with a history of a large first baby. She attended clinic regularly, went into labour, had a failed vacuum delivery and finally a forceps deliver and was delivered of a 6.1 kg baby with an Apgar of 0. The baby survived; she is severely mentally retarded.
